TERMS OF SERVICE.

Please ensure all new patient registration forms are filled out and sent back 24 hours prior to attending your consultation (exceptions made for last minute bookings). Forms that are not returned within this time frame may result in your appointment time being rescheduled.

If you can not attend an appointment please provide 24 hours notice so your appointment can be allocated to another patient. A 24 hour cancellation policy applies where a late cancellation will attract a $50 fee.

Any cancellations within 2 hours of your appointment will incur a fee to the total value of the appointment.

Bookings for Initial and Follow-up consultations are paid in full via cash or EFTPOS on the day of the consultation. Bookings for packages are paid online at time of booking.

COMMUNICATION BETWEEN APPOINTMENTS.

Like a conventional medical practice with a general practitioner, most of our contact should occur during your appointments.

Exceptions to this are any urgent enquiries, which may include things such as:

  • you do not understand something in the treatment notes I have provided
  • you are unable to arrange the testing we agreed on and need an alternative
  • you have a suspected adverse reaction to treatment and do not know whether to continue the treatment
  • you are having trouble ordering your supplements online

Examples of when you should book a follow-up appointment:

  • you have test results that you would like reviewed
  • another practitioner, friend, colleague or sales assistant has suggested a treatment, supplement or product and you would like my opinion
  • if you wish to continue your current supplements but your prescription has expired

PRIVACY POLICY.

This is the Privacy Policy of Lauren Howe (ABN 14 367 534 396), naturopath and nutritionist. I am  committed to providing quality services to you and this policy outlines our ongoing obligations to you in respect of how we manage your Personal Information.

I have adopted the Australian Privacy Principles (APPs) contained in the Privacy Act 1988 (Cth) (the Privacy Act). The NPPs govern the way in which I collect, use, disclose, store, secure and dispose of your Personal Information.

Please read this policy carefully. By providing your personal information to me, you consent to me collecting, holding, using and sharing your personal information in accordance with this policy.

What is Personal Information and why do I collect it?

Personal Information is information or an opinion that identifies an individual.

Examples of Personal Information I collect include:

  • name, date of birth, age, gender, occupation or job titile
  • addresses, email addresses, landline, mobile and facsimile numbers.
  • your health and lifestyle information including previous and current health and medical history, medications or current treatments, social history, family history and risk factors;
  • details of the products and services you have enquired about or purchased from me

This Personal Information is obtained in many ways including when

  • you purchase a product or service from me, including when you make a booking and during consultations;
  • you correspond with me on a social media platforms such as Facebook, LinkedIn, Instagram or similar sites;
  • you contact me with a question, comment or inquiry;
  • you provide information relating to your business; or
  • you visit my website.

I collect your Personal Information for the primary purpose of providing services to you, providing information to clients and marketing. I may also use your Personal Information for secondary purposes closely related to the primary purpose, in circumstances where you would reasonably expect such use or disclosure. You may unsubscribe from my mailing/marketing lists at any time by contacting us in writing.

When I collect Personal Information I will, where appropriate and where possible, explain to you why I am collecting the information and how I plan to use it.

Cookies

I may use cookies on my website. Cookies, by themselves, do not tell me your email address or other personally identifiable information. However, they help with things like site navigation and user-friendliness.

The majority of web browsers accept cookies automatically.  You can disable cookies, but it might restrict your ability to access certain areas of the Site.

Sensitive Information

Sensitive information is defined in the Privacy Act to include information or opinion about such things as an individual’s racial or ethnic origin, political opinions, membership of a political association, religious or philosophical beliefs, membership of a trade union or other professional body, criminal record or health information.

Sensitive information will be used only:

  • For the primary purpose for which it was obtained
  • For a secondary purpose that is directly related to the primary purpose
  • With your consent; or where required or authorised by law.

Third Parties

Where reasonable and practicable to do so, I will collect your Personal Information only from you. However, in some circumstances I may be provided with information by third parties. In such a case we will take reasonable steps to ensure that you are made aware of the information provided to myself by the third party.

Disclosure of Personal Information

Your Personal Information may be disclosed in a number of circumstances including the following:

  • Third parties where you consent to the use or disclosure; and
  • Where required or authorised by law.

Security of Personal Information

Your Personal Information is stored in a manner that reasonably protects it from misuse and loss and from unauthorized access, modification or disclosure.

As such, I have put in place reasonable physical, electronic and administrative procedures to safeguard and secure your personal information and protect it from misuse, interference, loss and unauthorised access, modification and disclosure.

For example, I maintain my files in secure offices and limit access to personal information to individuals with a need to know. I do use cloud storage for personal information with SimplyBook.me. These companies are subject to encryption and protection that can be found on their websites. If you are in communication with me via electronic means such as email, Skype, Zoom, contact forms, Instagram, LinkedIn or Facebook, I do not have full control over the transmission or storage or any personal information disclosed. You agree that by participating in such forms of communication you understand and accept that there is an inherent risk of disclosure or loss of your personal information for which I cannot be held responsible.

When your Personal Information is no longer needed for the purpose for which it was obtained, we will take reasonable steps to destroy or permanently de-identify your Personal Information. However, most of the Personal Information is or will be stored in client files which will be kept by us for a minimum of 7 years.

Access to your Personal Information

You may access the Personal Information we hold about you and to update and/or correct it, subject to certain exceptions. If you wish to access your Personal Information, please contact us in writing.

I will not charge any fee for your access request, but may charge an administrative fee for providing a copy of your Personal Information.

In order to protect your Personal Information we may require identification from you before releasing the requested information.

Maintaining the Quality of your Personal Information

It is an important that your Personal Information is up to date and I will take reasonable steps to make sure that your Personal Information is accurate, complete and up-to-date. If you find that the information I have is not up to date or is inaccurate, please advise me as soon as practicable so I can update my records and ensure I can continue to provide quality services to you.
